本人就授权“摄影者/录影摄制者”拍摄及使用本人肖像事宜，声明如下：
A hereby authorize“PHOTOGRAPHER/FILMMAKER” to film and use my likeness, and declare as follows:
1. 定义
Definitions
1.1 “模特”指本人，包括本人的外貌、外观和体型。
“MODEL” means myself, including my physical appearance and body form.
1.2 “媒体”指所有的媒介，包括数字、电子、印刷、电视、电影或其他已知或未知的媒介。
“MEDIA” means all media,including digital, electronic, print, television, film and other media known or to be invented.
1.3 “摄影者/录影摄制者”指摄影者、图形设计者、录影摄制者、电影摄影者或任何其他对本人进行摄影或录制的人或实体。
“PHOTOGRAPHER/FILMMAKER” means photographer, graphic designer, filmmaker or cinematographer, or any other person or entity who photographs or records me.
1.4 “受让人”指摄影者或录影摄制者转让或许可使用肖像的任何人或公司及其被许可人。
“ASSIGNS” means any person or any company to whom a photographer or filmmaker transfers or licenses a portrait and its licensees.
1.5 “肖像”指对本人的所有的照片、录影或录像，含其中的本人声音、表演。
“IMAGES” means all my photographs, film or recording, including the individual's voice and performance therein.
1.6 “对价”指本人因授予使用肖像权而获得的报酬。
“CONSIDERATION” means rewards I have received in exchange for the rights granted by me in this release.
1.7 “拍摄”指本授权书中描述的摄影或录影活动。
“SHOOT” means the photographic or film session described in this form.

Content of License
本人已经确认收到了良好有效的对价，并通过签署本授权书，许可摄影者/录影摄制者和受让人在任何媒体上或出于任何目的（除了色情或诽谤性的） 使用或授权他人使用本人肖像， 包括对任何产品或服务所做的广告、促销、营销和包装。本人同意肖像可与其他图像、文本和图形相结合，也同意肖像可被剪切、变更或修改。
A have acknowledged receipt of good and valid consideration and, by signing this authorization, I hereby give the Photographer/Filmmaker and assigns my permission to use or authorize others to use the Images in any Media for any purpose（except pornographic or defamatory）including advertising, promotion, marketing and packaging of any product or service. I agree that the Images may be combined with other images, text and graphics, and can be cropped, altered or modified.
3. 陈述并保证
Representations and Warranties
在整个拍摄期间，本人已年满18周岁或根据拍摄地的法律规定已经成年，并具有完全行为能力签署本授权书。如模特是未成年人或依照拍摄地的法律没有民事行为能力，则其监护人陈述并保证其为模特的合法监护人，拥有完全的民事行为能力，以此同意进行拍摄并签署本模特肖像使用授权书。如监护人同意拥有上述的民事行为 能力，请将监护人的姓名打印于下述签字栏内，并代表模特签署本授权书。
At the time of the Shoot, I am at least 18 years of age or the age of majority under the laws of the jurisdiction where the Shoot occurs, and have full legal capacity to execute this release. If Model is a minor or lacks capacity in the jurisdiction of the Shoot, a guardian warrants and represents that he/she is the legal guardian of Model and has the full legal capacity to consent to the Shoot and to execute this release OF ALL RIGHTS IN MODEL’S CONTENT. If you are signing in this capacity, please print your name below and sign the document on behalf of the Model.
4. 附则
Supplementary Provisions
本人承认对上述肖像不存在任何权利，肖像的所有权利均归摄影者/录影摄制者和受让人所有。本人承认并同意无权要求额外的报酬和费用，且不得以任何理由向许可摄影者/录影摄制者和受让人提出进一步的权利主张。本人同意，本授权书对本人的继承人和受让人同样具有约束力。本授权书是不可撤销的，并受拍摄地的法律管辖。
A agree that I have no rights to the Images, and all rights to the Images belong to the Photographer/Filmmaker and Assigns. I acknowledge and agree that I am not entitled to additional consideration or accounting, and that I will make no further claim for any reason to Photographer/Filmmaker and/or Assigns. I acknowledge and agree that this release is binding upon my heirs and assigns. I agree that this release is irrevocable and will be governed by the laws of the jurisdiction of the location of the Shoot.
